High cholesterol is often dubbed the “silent killer” because it progresses for years without clear symptoms, until serious health problems arise. Still, your body can offer visual signals of elevated cholesterol long before a cardiovascular crisis. Subtle skin changes, eye anomalies, unexplained fatigue, and limb discomfort may all be red flags. While the definitive diagnosis comes from a blood lipid panel, being alert to these visible clues allows individuals to take proactive steps. Recognizing these signs early – like yellow eyelid patches or grayish eye rings – can empower you to seek timely testing and treatment.


Here are seven visible yet often overlooked signs of high cholesterol, along with what each might reveal about your cardiovascular health.

Yellowish deposits on eyelids – Xanthelasma

One of the most recognizable signs of high LDL (‘bad cholesterol’) is xanthelasma – pale yellow plaques appearing along the inner corners of the eyelids. These are soft cholesterol-rich deposits beneath the skin. Though benign and painless, they often signal systemic lipid imbalances and are more common in individuals with familial hypercholesterolemia. Since they indicate elevated LDL, noticing them should prompt a cholesterol test and cardiovascular risk assessment.

White or gray rings around the eyes – Arcus Senilis

Another ocular sign, arcus senilis, presents as a gray or white ring around the cornea's edge. While a natural aging effect, early-onset arcus (before age 45–50) is often linked to high cholesterol. Lipid deposits accumulate around the cornea's periphery and, though vision remains unaffected, the condition suggests lipid metabolism irregularities that may require medical follow-up.

Yellowish bumps on skin and tendons – Xanthomas

Raised yellowish bumps on the skin, called xanthomas, are cholesterol-rich deposits that appear on bothers like elbows, knees, hands, feet, or buttocks. These nodules can be small or clustered and often accompany more serious familial lipid disorders. In notable cases, individuals consuming extremely high-fat diets developed widespread, painless yellow nodules with total cholesterol levels exceeding 1,000 mg/dL.

Tendon thickening or nodules – Tendon Xanthomas

Cholesterol may deposit in tendons, creating firm, painless nodules – tendon xanthomas – notably on the Achilles, hands, or elbows. These signs are particularly indicative of familial hypercholesterolemia. Their presence often correlates with very high LDL cholesterol and deserves aggressive medical evaluation.

While precise evidence is more limited, some additional observations may hint at lipid issues, such as:

Erectile dysfunction – Poor vascular function from cholesterol can reduce circulation to the penile tissues.

Gallbladder pain – High cholesterol can lead to gallstones, causing upper abdominal discomfort.

Joint pain or gout – Although not directly caused by cholesterol, metabolic imbalances in lipid disorders may trigger joint inflammation.

Memory and concentration problems – Reduced brain perfusion may manifest as cognitive fog or difficulty focusing.
