4 simple exercises that can help relieve foot pain from diabetic neuropathy

Diabetic peripheral neuropathy is a common complication of diabetes, often marked by pain, numbness, weakness and poor foot‑ankle mobility that can seriously impair daily function and quality of life. Recent research increasingly shows that, beyond medication, targeted physical exercises for the foot, ankle and lower leg offer a promising, non‑pharmacological strategy to support nerve and musculoskeletal health in people with DPN.
What makes these findings especially compelling is that many of these positive effects appear even with simple, low‑intensity exercises. As such, exercise emerges not as a cure, but as a practical, accessible and evidence‑supported component of neuropathy self‑care.

What is diabetic neuropathy and what are its risks

​Diabetic neuropathy refers to nerve damage caused by diabetes, resulting from chronically elevated blood sugar and associated metabolic disturbances. This means the nerves that normally carry signals for sensation, movement, and regulation of organ systems, become impaired.

Since nerves control sensation, muscle function, and organ regulation, diabetic neuropathy can lead to a variety of serious complications:
- Loss of protective sensation can lead to unnoticed injuries, infections, and even amputations.
- Nerve damage in legs or feet can lead to motor or balance impairment, and may increase risk of falls, gait problems, reduced mobility.- Many people experience burning, tingling, stabbing pain, or other sensory disturbances, which is a sign of chronic neuropathic pain.

Why exercise helps neuropathy

Multiple studies show that regular, structured movement can improve nerve health, foot function, and pain. Here are the mechanisms supported by research:
- it improves nerve regeneration and nerve fiber density
- Exercise strengthens foot and lower-leg muscles weakened by neuropathy
- Exercise improves gait and reduces fall risk
- Exercise supports better blood-glucose control, slowing further nerve damage

Below are four simple exercises that can help relieve foot pain from diabetic neuropathy

Heel-toe raises

Stand behind a chair and hold for support, or sit on the chair.
Lift heels off the ground so you’re on your toes, then lower.
Reverse by lifting toes while heels stay down.

A 2021 study compared people with diabetes + peripheral neuropathy (DMPN) and controls, assessing foot and ankle motion during unilateral and bilateral heel‑rise and non‑weight bearing plantar-flexion tasks. It found that people with DMPN had reduced plantar‑flexion (heel‑raise) range of motion at the foot and ankle compared with controls.

Standing heel raises

Stand upright with feet hip-width apart, take support, if needed.
Slowly lift one heel, while keeping toes on the ground.
Lower the heels back to the ground.
Repeat the same exercise with the other leg.

A 2021 study compared 60 people with diabetes and peripheral neuropathy and 22 non-diabetic controls. It measured foot and ankle plantar-flexion during three tasks: unilateral heel-rise, bilateral heel-rise, and non–weight-bearing ankle plantar-flexion. The study found that heel-rise is feasible for people with neuropathy, but performance (such as; range of motion, strength) tends to be reduced. This suggests that such exercises can be used, but must be adjusted for limited mobility and monitored carefully.

Forward leg stretch

Sit on a chair with both legs on the floor.Extend one leg parallel to the floor, and hold for 1-2 seconds.Gently put the leg down on the floor.Repeat the same with the other leg.

A randomized controlled trial of combined strengthening, stretching, and functional foot-ankle training in patients with Diabetic Peripheral Neuropathy (DPN) found improvements in foot and ankle function, better “foot-rollover” during gait, improved ankle dorsiflexion and toe movement, and better overall foot-ankle muscle performance.

Leg march

Sit on a chair for safer practice.
Lift one leg toward hip level, then lower it.
In a marching motion, lift the other leg similarly.
Create an alternating marching motion.

A 2012 pilot study of people with DPN who participated in a 10-week aerobic + resistance training program reported reductions in neuropathic symptoms and pain, and, notably, an increase in peripheral nerve fiber branching from skin biopsy.
